Missed Flu Immunisation’s.

Good Morning

If your child missed the flu immunisation session in school and you wish for them to have it, please follow the below instructions.

Follow the link below that was sent out in the original notification. This opens up a letter from the Leeds community health team. Within that letter is a link to click on which takes you to the consent form.

The school code you need is also contained within the letter.

Complete the details for your child (you must do one per child). 

You then need to ring 0113 8433101 to book a session for your child.

Please note the nurses do not automatically return to school. You must also ring once you have completed the consent, to arrange your child’s vaccine.
